We ask each of our clients to fill out several forms and questionnaires that share with us their Health History, goals, obstacles, etc. This information assists us in coaching and training our clients so they can reach their goals, be confident and successful with their training program.

If you are a new client please download these forms and answer them as honestly as you can. Take some time in working on your goals and strategies form for these will be the building blocks of your program.
